Shadowing

In order to shadow at UPMC, you must complete OMED paperwork and training. PLEASE READ THE ENTIRETY OF THE DIRECTIONS — THEY ARE VERY IMPORTANT!!!

Please fill out this form before submitting your paperwork. If you do not fill it out prior to applying for shadowing, you will not be considered for the opportunity you apply for.

Students have to get credentialed first, then attend an orientation meeting with UPMC.

• UPMC offers orientation meetings twice a month.

• Students will be emailed directions for scheduling an orientation.

Credentialing Steps

• Fill out the packed linked below!

  • You must print this packet out, hand sign it, then scan it

  • YOU CAN NOT USE AN E-SIGNATURE

  • SUBMIT IN ONE COHESIVE DOCUMENT AS A PDF!

    • NOTHING ELSE WILL BE ACCEPTED!

• TB Test Requirements

  • You must obtain either a Quantiferon Gold Blood test or a T-Spot Blood test

  • YOU CAN NOT GET A SKIN TEST

  • You can get it from Student Health, UPMC, or any other lab

    ■ If obtained from UPMC or student health, the TB test is good for 4 years of OMED

  • You can use a test if it is a year old and it will be valid for this year of shadowing

    ■ However, you must get a new test for next year

  • Ways to pay:

    ■ UPMC will bill insurance and usually, your insurance will cover it if you haven't gotten a TB test in a year

    ■ If not, you get billed in your student account for ~ 90 bucks

• Flu shot Requirements

  • Valid until Nov 1 if you got it last year

  • Need the new one for this season

  • You can get it at most pharmacies, UPMC, and Student Health

  • Need a screenshot or picture of the information given to you

  • Make sure you ask the people who vaccinated you for documentation

• Credentialing Packet Notes

  • Fill out as is and specify that you are shadowing with AMSA

  • For the page with symptoms, just mark it as you feel now

  • PLEASE INCLUDE YOUR TB TEST AND FLU SHOT IN THE PDF WITH THE REQUIRED PAPERWORK!!!

• Notes!

  • Don’t throw out any vaccination results/documentation just in case!

  • Email VCR for any questions! DO NOT EMAIL QUESTIONS TO UPMC!!

  • After submission, OMED will verify your information

  • Students will get an email back to schedule an orientation meeting

  • After orientation, Dr. G will confirm everything and email us about the credentialing results
